I hereby acknowledge and agree:

The purpose of the Epigenetics Coaching and/or Peptide Coaching Services (collectively, the "Coaching Services") is to improve the overall health, vitality, and well-being of the body and mind through nutritional education, epigenetic/DNA-based lifestyle education, NLP (Neuro-Linguistic Programming) techniques, hypnosis work, somatic practices, and the use of natural foods and non-medicinal nutritional supplements. Cara Barone LLC, aka Cara Barone (hereafter "Coach"), does not diagnose diseases, disorders, or conditions.

Where Peptide Coaching is part of the Coaching Services, I acknowledge and understand that peptides are NOT approved by the U.S. Food and Drug Administration and are considered to be for research purposes only.

Coach does not prescribe, dispense, administer, or provide dosing instructions for any substance, and any decision to source, obtain, or use peptides is made entirely at my own discretion and risk.

Coach is neither required to be, nor is, a licensed Dietitian, Naturopathic Doctor, or Medical Physician, unless otherwise specified in writing.

As part of the Coaching Services, I may be asked to provide information concerning my physical habits, medical history, genetic/DNA test results, moods, energy levels, likes and dislikes, lifestyle, and diet. This information is collected to enable Coach to: (i) assess my knowledge of nutrition and lifestyle factors, (ii) educate me about the benefits of sound nutritional and lifestyle practices, and (iii) recommend dietary and lifestyle changes to improve my general health, vitality, and overall well-being. Coach will hold this information in confidence and will not release or disclose it to any other person without my prior consent, except as required by applicable law.

If Coach suspects the existence of a disease, disorder, or condition, I will be informed of this suspicion. I acknowledge this is not a diagnosis or conclusion about the state of my health, and that I am directed to promptly consult a licensed Physician or Naturopath about any suspected problems.

Coach is not a substitute for medical care from a licensed provider. I acknowledge that no information received from Coach is considered a diagnosis or conclusion about the state of my health, and that I am directed to promptly consult a licensed Physician about any suspected problems.

Should I request Coach to recommend dietary, lifestyle, or supplement changes to enhance my body's natural ability to resist and/or overcome a known disease, disorder, or condition, it is my responsibility to disclose the nature of that disease, disorder, or condition and all other relevant details to Coach.

If I have not previously consulted a licensed Physician or Naturopath about this disease, disorder, or condition, I acknowledge that I am directed to promptly do so. I am not to alter or discontinue treatments prescribed by a licensed Naturopath, Physician, or other licensed health professional without first consulting the individual who prescribed the treatment.

In providing the Coaching Services to me, Coach is relying upon the truth, accuracy, and completeness of all information I have provided. Any recommendations I follow for changes in diet, lifestyle, or the use of nutritional supplements, are entirely my responsibility.

If I experience a medical emergency or believe I am having a serious adverse reaction at any time, I understand that I should immediately call 911 (or my local emergency number) or seek emergency medical care, rather than contact Coach.

I understand that the Coaching Services, including any NLP, hypnosis, or somatic work, do not constitute an attempt to practice trauma or clinical mental health therapy. I acknowledge that I should consult a licensed clinical mental health care provider for mental health advice and for answers to personal mental health questions, especially regarding (but not limited to) traumatic events.

I understand that information shared by Coach is not to be considered mental health advice and is not intended to replace consultation with a qualified mental health professional, and that Coach and Coach's team of employees or contractors do not answer specific mental health questions.

Neither Coach nor Cara Barone LLC is in any way liable for my health or safety.

In consideration of my participation in the Coaching Services, I hereby accept all risk to my health, including injury or death, that may result from such participation, and I hereby release Coach and Cara Barone LLC, on my behalf and on behalf of my personal representatives, estate, heirs, next of kin, and assigns, from any and all costs, claims, causes of action, and damages arising from any and all illness or injury to my person, including my death, that may result from or occur as a result of my participation in the Coaching Services, whether caused by negligence or otherwise.

I understand that any therapies, protocols, or recommendations I undertake through Cara Barone LLC are undertaken of my own free will. I accept that the ultimate responsibility for my health care is my own, and that Cara Barone LLC is here to support me in this.

I understand that Coach reserves the right to determine which cases fall outside Coach's scope of practice, in which event an appropriate referral will be recommended. I hereby agree to assume full responsibility for any manner of loss, injury, claim, or damage whatsoever, known or unknown, incurred as a result of same, and I, my heirs, executors, administrators, or assigns waive any claim for any loss, injury, claim, or damage sustained as a result of my attendance and/or participation.

I have read the above release and waiver of liability, and fully understand its contents and voluntarily agree to the terms stated.

I acknowledge that all materials, resources, protocols, trainings, and content provided as part of the Coaching Services are the intellectual property of, and are All Rights Reserved to, Cara Barone LLC, and should not be shared, copied, or distributed without express written permission from Cara Barone. I agree to access these materials for my personal use only.

This Agreement shall be governed by the laws of the State of Florida, USA, without regard to conflict-of-law principles, regardless of my country of residence. I agree that any dispute arising from or relating to the Coaching Services shall be brought exclusively in the state or federal courts located in Florida, USA, and I consent to the personal jurisdiction of those courts. If any provision of this Agreement is found unenforceable under the law of my country of residence or any other jurisdiction, the remaining provisions shall remain in full force and effect.

I acknowledge that I am solely responsible for complying with the laws of my own country of residence regarding the receipt, importation, possession, and use of any peptides, supplements, or other products discussed as part of the Coaching Services, including any customs, import, or controlled-substance regulations that may apply where I live. Coach makes no representation as to the legality of obtaining or using such products outside the United States.

Payment Terms
By purchasing the Coaching Services, I agree to pay the full amount owed, in USD. If I am using a payment plan, I am obligated to pay as agreed, on the dates agreed upon, and in full, regardless of how I utilize the program or whether I choose to participate. All deposits and payments are non-refundable, regardless of how the program is utilized, including if I choose not to participate. This no-refund policy applies equally to pay-in-full payments and payment plans.
Payment-plan installments will be charged monthly on the date of original purchase, and it is my responsibility to track this date and ensure sufficient funds are available.

Services Provided
Coach agrees to provide services as deemed appropriate, which may include NLP techniques, hypnosis work, somatic practices, and related coaching resources such as PDFs, worksheets, homework, audio recordings, and external links. Coach will make reasonable efforts to respond to client communications within the timeframe specified for my service tier, excluding public holidays, notified vacations, and weekends.

Personal Responsibility
I agree that once I have purchased the Coaching Services, it is my responsibility to review onboarding materials, emails, and provided links, and to take full personal responsibility for completing what is required. I understand that Coach will provide tools, guidance, coaching, materials, and support, but that I am ultimately responsible for creating any change, results, or transformation in my life, health, relationships, fitness, or business. I understand that Coach makes no guarantees regarding results, and that any testimonials or examples shared are not promises of results for my particular situation.

I HAVE CAREFULLY READ THIS AGREEMENT AND AGREE TO THE TERMS OUTLINED ABOVE. I UNDERSTAND THIS AGREEMENT TO BE A FULL AND FINAL RELEASE OF ALL COSTS, CLAIMS, CAUSES OF ACTION, AND DAMAGES OF ANY KIND ARISING FROM OR IN CONNECTION WITH THE COACHING SERVICES.
